试述肩关节前脱位的诊断?

患者有明显外伤史或既往有习惯性肩关节脱位史,稍受外力作用又复发。脱位后肩部疼痛、肿胀、功能障碍。患肩失去圆形膨隆外形,肩峰显著突出,肩峰下部空虚,形成"方肩"畸形,在喙突下,腋窝内或锁骨下可触及肱骨头,盂下脱位时患肢较健侧长。肩关节弹性固定于肩外展20-30度的位置。搭肩试验阳性。X线检查可了解肱骨头移位的方向与位置,确定脱位的类型,并可了解有无并发骨折。
