(1)感染来源以龋病、牙周膜炎、根尖感染为主;
(2)感染途径是先破坏骨髓,后破坏骨皮质,再形成骨膜下脓肿或颌周间隙感染。因此,骨髓质与骨皮质多同时受累;
(3)临床表现可以是局限型,但以弥散型较多;
(4)骨髓炎病灶区周围牙松动,牙周有明显的炎症;
(5)病变多在颌骨体,也可以波及下颌升支;
(6)慢性期X线所见病变明显。可以有大块死骨块,与周围骨质分界清楚,或伴有病理性骨折。
边缘性颌骨骨髓炎:
(1)感染来源以下颌智齿冠周炎为主;
(2)感染途径是先形成骨膜下脓肿或颌周间隙感染。主要破坏骨皮质,很少破坏骨髓质;
(3)临床表现多系局限型,弥散型较少;
(4)骨髓炎病灶区周围牙及牙周组织无明显的炎症;
(5)病变在下颌角及下颌支,很少起于或波及颌骨体;
(6)慢性期X线所见病变多系骨质疏松、脱钙或骨质增生,或有小块死骨,与周围骨质无明显分界。
