何谓新生儿生理性黄疸?试述其发病机制。

新生儿(特别是早产儿)出生后多在最初几天内发生轻度的非酯型高胆红素血症和一时性黄疸,1~2周后逐渐消退,这种黄疸称为新生儿生理性黄疸。其发病机制为:

①新生儿肝细胞合成胆红素葡萄糖醛酸基转移酶的功能不成熟,以致肝脏不能充分酯化胆红素;

②在新生儿期,肝细胞合成Y蛋白相对不足,使肝细胞对胆红素的摄取-运载过程减慢;

③新生儿期都有一时性红细胞急速破坏,使肝细胞的胆红素负荷增加。
