简述子宫肌瘤的鉴别诊断。

(1)妊娠子宫:妊娠有停经史,早孕反应,子宫随停经月份增大变软,借助尿或血HCG测定、B超检查多可明确。

(2)卵巢肿瘤:多无月经改变,肿物位于子宫一侧,借助B超、腹腔镜等检查多可明确。

(3)子宫腺肌病:可表现为月经增多,子宫增大,但常有继发性渐进性痛经史,子宫多均匀增大,很少超过3个月妊娠大小,质硬有压痛。B超有助于诊断,病理检查可明确。

(4)子宫恶性肿瘤

①子宫肉瘤:好发于老年妇女,生长迅速,活组织检查有助于鉴别。

②子宫内膜癌:好发于老年妇女,主要表现为绝经后 * * 流血,子宫呈均匀增大或正常,质软。分段诊刮有助于鉴别。

③宫颈癌:有不规则 * * 流血、接触性出血等表现。可借助B超、宫颈细胞学刮片检查、宫颈活检等鉴别。
