左心室肥大(图6-1-19):必须具备QRS波群电压增高的一项标准,V5或V6导联的R波>2.5mY;Rv5+Sv1>4.0mY(男),或3.5mV(女);Ravl>1.2mV;RI>1.5mV;RavF>2.0mY;RI+SⅢ>2.5mV。

临床资料:男性,26岁。主动脉关闭不全。

心电图分析:P-R=120ms,QRS=92ms,Q-T:354ms。P、R、T电轴+5。、+40。、+9。。V1~V6导联q波逐渐加深,V5导联R波=3.7mV,胸壁导联ST段呈上斜型抬高0.10~0.20mY,V1、V2导联T波直立。提示左室舒张期负荷增加。

心电图诊断:①窦性心律:②左心室肥大。

可出现额面心电轴左偏。室壁激动时间在V5或V6导联超过0.05s。

QRS波群时间延长,多不超过0.12ms,R波为主的导联上,ST段可呈下斜型压低达0.05mV以上,T波低平、双向或倒置。以S波为主的导联上,T波反而直立。QRS波群电压增高同时有ST-T改变者,称左室肥大伴劳损。

右心室肥大(图6-1-20):V1导联R/S≥1,V5导联R/S≤1或S波比正常加深,重度肥厚可使V1导联呈qR型(除外心肌梗死)。

RVv1+Sv5>1.05mV(重症>1.2mV);aVR导联的R/q或R/s≥1,R波>0.5mV。心电轴右偏≥+90°(重症可>110°)。

临床资料:男性,23岁。先心病,肺动脉重度狭窄。

心电图分析:P-R=148ms,QRS=104ms,QT/QTe=348/406ms。P、R、T电轴+5°、+120°、-23°,V1导联呈qR型,V1导联R波=3.0mV,V2导联R波=4.6mV,V5、V6导联以s波为主,显示右室肥厚的特征。

心电图诊断:①窦性心律;②右心室肥大。

室壁激动时间在V1或V2导联超过0.04秒。

ST-T改变,同时合并右胸导联(V1、V2)的T波双向、倒置、ST段压低等,称右室肥大伴劳损。

诊断右室肥大,有时定性指标(如V。导联QRS波形态及电轴右偏等)较定量指标更有意义;心电图对右心肥大的诊断敏感性较差;心电图诊断的心室肥大与实际解剖之间有时有差距,临床判断时应综合分析。
