(1)分型：较通用的分类法为5级分类法，免疫力较强的结核样型麻风(TT)为一端，将免疫力较弱的瘤型麻风(LL)为另一端，在两端之间为免疫力不稳定的界线类偏结核样型麻风(BT)、中间界线类麻风(BB)和界线类偏瘤型麻风(BL)。这是根据机体免疫力由强到弱、麻风杆菌数量和类型演变来分型，又称为免疫光谱分类法。总的趋势是：麻风杆菌数量LL>BL>BB>BT>TT，而细胞免疫反应强度TT>BT>BB>BL>LL。麻风早期为未定类麻风(IL)，可演变成免疫光谱中的任何一个类型，也可自愈。细胞免疫力增强时BL可向结核样型端转化(BL→BB→BT)，反之BT可向瘤型端转化(BT→BB→BL)。
