患儿，男，6天，汉族。因“生后面色发绀4天，反应差、拒乳2天”入院。患儿系第一胎第一产，孕39周，胎膜早破24小时，剖宫产娩出，出生体重3800g，出生时Apgar评分1分钟9分（肤色扣1分），5分钟9分，羊水Ⅲ0污染，患儿生后面色发绀，3小后开奶，哺乳欠佳，分娩医院给予吸氧，静点“青霉素”治疗，效果不佳，生后2天出现拒乳，反应差。病程中无发热、抽搐，大便为墨绿色胎便，小便量少。查体：T不升，P130次/分，R40次/分W3500g。一般状态差，发育中等，全身皮肤黄染，无出血点。前囟平软，双侧瞳孔等大等圆，对光反射灵敏，球结膜无水肿及充血，口周明显发绀。颈软，无抵抗。听诊双肺呼吸音粗，可闻及细湿性啰音。心音有力，心率130次/分，律齐，各瓣膜听诊区未闻及杂音。腹部平坦，触之软，肝于右肋下2．0cm，剑下3．0cm，脾左肋下2．0cm，可触及，肝脾质地稍硬，肾脏未触及，肠鸣音正常。四肢凉，四肢肌张力减低。原始反射未引出。辅助检查：血常规：RBC3．9×10⁹/L，Hb168g/L，WBC6．5×10⁹/L，N：0．65，St：0．05，L：0．30。
